Guys,

Have seen some very good discussions and work here.
By any chance, if anyone has performed any sort of LOPC analysis, just share something which may help.
Need to just read if there is already some good work done out there.
Thanks.

There is a publication available from CCPS, "Methods to Analyze Loss of Containment Scenarios".
I would recommend you to apply those methods. Pretty useful as we used them when analyzing our case.

jeem,
I am not sure if ww2i has been understood correctly.
The publication you are recommending is the one considering LOPC scenarios while performing a process hazard analysis, and ww2i may not be looking for that.
What I understand is that he needs some kind of an analysis which might have been performed for actual LOPC cases happened at site.

thank you, and that is exactly what I am looking forward to that how a good analysis could be made.
Any pointers actually would support.

If you ask me, this is quite limited.
Give me some time and I would make sure to bring in some useful info from my colleagues responsible to analyse leaks.

Alright so just to have correct analysis and learning in place from LOPCs, following is recommended as I discussed with Operations, Maintenance and my HSE colleagues:
1. Source identification
2. Repetition case identification
3. Case to case investigation
4. Determination of quantity of release /how big was the leak
5. Identify the discipline handling the process containment
6. Issuing an initial report for all cases
7. Running such an analysis on monthly basis
8. Tracking of actions already identified
9. Inspection data having potential to turn to a leak in coming running time
