We have been tasked to perform a gap analysis of currently applied LOTO process at our site.
Thought to establish a checklist which could provide reference based checks to be performed to ensure current compliance.
Any notes would be helpful.

Like any audit, you need to start with the controlling document requirements.
OSHA 1910.147 is the recommended standard as the baseline check.
I have a checklist attached with this post but basically, the following checks are required to be performed under these categories:

• Governance & Documentation
  • Energy Source Identification
  • Isolation Devices & Hardware
  • LOTO Application
  • Verification of Isolation
  • LOTO Removal
  • Training & Competency
  • Auditing & Improvement
