Dynamic Risk Assessment

We have been advised by our HSE colleagues to apply dynamic risk assessment especially when there is a hazard identified during an on-going activity which was not anticipated before.
Is that the only criteria when a dynamic risk assessment should be initiated?
Do we have some other indicators as well necessitating that?

I will take this as well.
Dynamic risk assessment is connected with identification of a hazard/s once the job is in-progress.
We opened up a heat exchanger that was planned to be cleaned with hydrojetting however, the deposits were not getting cleaned and the pressure was being increased. It was identified that the job is required to be reviewed before any increase in pressure is applied. A case of a dynamic risk assessment, happened recently.

Understandable, but the current challenges we face are:
1. Awareness of personnel performing the job to recognize the unidentified hazard
2. Stop and taking the charge to report
3. Job continuation impact
There has been a scenario where a rigger noticed that the approved lifting plan is not working to lift a PSV. This was qualifying a clear case of a dynamic risk assessment, stop the activity, inform the job supervisor, and revise the plan with the lifting engineer.
Instead, he took the decision of continuing as that was towards the end of the shift, resulting in PSV hitting the nearby asset, potentially causing damages. During the assessment, it was identified that if dynamic risk assessment would have been applied, incident might not have happened, which we all agree.
Behavior part of the issue requires deeper attention, in my opinion. System helps afterwards.
