Risk Assessment Practices

We practice performing risk assessment for plant related activities but not including all of them. Regular preventive maintenance jobs are exempted however activities being performed as part of change /modification being carried out by contractors are supported with risk assessment. What is the criteria here? Which plant activities should be supported with risk assessment and which not?

The practices which we follow at our site is that any new activity linked to a change has to undergo risk assessment + when a hazard is identified during the job (not previously assessed for the risk) + any new activity (outside change management like performing robotic inspection).
All in all three scenarios, as we practice.

Agreed with these three leads when a risk assessment MUST be performed.
How about handling the routine jobs, how the risk is assessed for them?

Routine activities are normally controlled with the existing HIRAs in place for them including sand blasting, scaffolding works, fabrication job, inspection works, greasing activities etc. These documents are required to be updated at an acceptable (3 years normally) frequency so that if there are any updates on the way these routine activities are performed, any new hazard can be identified and accordingly should be assessed for the risk and controls to be exercised.

Yes the similar practices as ivani1 has explained for routine activities subjected to a review by concerned. A complete list of maintenance routine activities are supported with HIRA which are validated by Maintenance Engineers, reviewed by HSE Engineers, and then made available for use.

neo & ivani1, inputs will help.
The process of having defined HIRA and their validation is a good practice.
With some activities being added directly by the executors with less information to Operations & HSE must be considered through spot checks to identify gaps proactively. This is what we observed. A risk assessment performed years ago for a job which has been added up with some new activities may require to be looked into by personnel other than the ones performing the job.
