Some process inputs needed on relief system venting directly to the atmosphere and that too horizontally.

Sincere thanks to @tuan creating this post and then to @mechcolor for the calendar.
Next is th eincident happened in Shell, Netherlands. A bunch of learning from there as well.
Alarm management being the key here if someone asks me and then the bypassing of safety critical trips, which should not be done while performing some planned preventive maintenance activities. Another pointer for me was the adequate manning.
