PMI Issue

During site inspection, Positive Material Identification revealed a non-conformance of welding material. As per drawings, the weld had to be in P-22 material but found to be P-11. It was recommended to cut the joint and re-weld with the right filling material.
Later, it was observed that the Contractor only removed the capping of the weld joint and started welding. Same was clarified by performing PMI on filling passes giving P-11 material. Full joint was not cut and only the capping area was being made with P-22 electrodes.
A very serious violation.
