Where there are chances of process blockages, it is recommended to install emergency relief devices upstream of the possible blockages. However, how these scenarios should be identified? Part of design stage HAZOP or through Process Engineering itself?

That sort of situation mandates that a careful analysis should be performed based upon historical industrial data, and personnel experience to once be sure about the blockage nature & probable location. PSV is then required to be installed upstream of that blockage location otherwise this last defence would be masked.
