As part of process safety metrics, safe operating limits excursions, everything is required to be reported, be it the flow, concentration etc.?

Yes that is correct. As per API 754, now 2026 one is out as well, there is no mention of any certain parameter. Every excursion /exceedance qualifies to be reported.
But the actual issue is that someone from Operations /Process should review that excursion to be a real one.
Some of those are happening just during the plant startup /shutdown and requires evaluation before declaration.

Yes this has to include all excursions expect those which were reviewed and evaluated not to be the ones happened during the normal operation.
