HSE Actions Implementation
We see that there are a lot of sources through which HSE critical recommendations are generated and they stand value to prevent any incident. Those sources include MOC, HAZOP, PSSR, IA, LOPA, Technical Studies, Risk Assessments, and many others. What is that ONE system where these actions should be well managed?

Let us talk from the excel tracking, the basic computer aided tracking.
We moved from there to a system build by our own IT guys. That system itself has been updated several times and now everything is there.
We keep on talking about critical & non-critical actions however we have agreed to keep all such HSE related actions posing at least Medium risk and we have this option to select also once initiating the action to tag if it is HSE related. There was a long debate within our process safety section and corporate not to have any process safety action as a Low risk. A lot many discipline engineers still do not agree with this approach.

Pain area, I would say.
You put one system in place, keep working on that, enhancing as well and then IT comes up with a proposal of a new system.
I mean honestly, it should be other way round. We know how best we can manage our actions and where.
In my experience, the system which stays long in use with people should be the one to remain in place for action tracking.
There are areas where innovation may likely to kill the essence of the subject.

Dashboards have been in the reports now a days to support implementation of agreed actions.
Companies are investing to have visibility of systems and the actions coming out of them.
Risk ranking these actions should be the basic thing to do. 800 actions in progress would not provide any idea where to focus.
Excel based tracking may not be viable now but the thing is that control was having an owner. Even for advanced dashboards, there should be personnel nominated to explain them.
