Noticing a lot of threaded joints failures from a lot many different sources including instrumentation connections, threaded plugs, fittings, ferrules.
What should we be looking into when considering all this?

There are lot many ways you can factor this issue to resolve.
I would recommend you that go to the level of an individual case identification with reference to its source, component, functionality and the actual site conditions to find something obvious. At this level, if there is something which is not identified you may go even further to assess whether the design was all good for these locations /conditions. And eventually, with frequent threaded joints failures, you need to identify if there is some recent works which have been performed to look into the workmanship /quality issue of the job being done. But in no case you start with this. Breakdown each case first to see what indicators come up.

To me, there has to be some issue with the team working on these threaded joints.
Just look in this way, design issue, that cannot be occurring on all the locations and that too simultaneously and frequently.
Even if this is related to some operational issue, we do not expect this to happen the way you explained, "a lot many different sources".
Did you lose some of your technicians /supervisors or new ones might have joined? You need to rule this out first before investing manhours to go further to investigate.
