When is it recommended & feasible to perform repair on piping flanges?
Brief me about the mandatory inspection which is required to be made when the flanged connections are opened up, and for which services?

Glad that you opened up this discussion.
I believe that most flange failures are not due to bad gaskets, they are due to damaged flange faces, reused bolting, or poor inspection discipline.
This points towards significance of flange inspection whenever there is an opportunity.
Changing the gasket, bolting up, and not assessing the condition of the flanges may lead to actual leaks once in service.
Remember API 570 explicitly talks about the inspection to be made on flanges.
And then API 581 describes them as:
High leak probability items
Often bad actors
Key contributors to Tier-1 / Tier-2 events

I believe I made the point here.

Practically speaking, how can flanges be inspected?
Should there be some kind of a maintenance plan?
Opportunity based? And if opportunity based, then how that should be utilized? What should be addressed?
For equipment, pressure vessels, heat exchangers, I can understand we have opportunity, and that is not missed to check the flange face condition.
What shll be done with piping?

neo, if you are looking for something which is mandated anywhere in the written standards /codes on inspection of flanges, let me tell you very clearly, such a requirement you would not be able to find, and implement.
So, what you can do here:
When flanges are opened, make inspection a mandatory, enforce that through a procedure at your site:
And then;
Apply VT for all services
Apply enhanced NDE for hydrogen, sour, lethal, cyclic, or high-risk services
Bolting and gasket inspection always!
