What are the reasons of gas lift valve malfunction, and how that can be avoided?

There can be many different scenarios when the gas lift does not function.
What exactly has happened in your case?
Has this been a total failure where the valve has not lifted at all, or lifted but got stuck or lifted at an incorrect depth?
All of these cases have their own causes of malfunction.

In our case, there was no lift at all happened.
We are looking into the causes thinking that there might have been no casing-to-tubing pressure response leading to this.
