These kind of scenarios happen and are regularly being reported by Operations, indicating that swift action is the one required.
At our site, there a lot of heat exchangers utilizing cooling water on one side. Leaking tube can end up having hydrocarbon in CW discharge side.
This has been a learning neo, thanks for sharing.
