What are the practical inspection programs with yourself all at your sites to handle CUF issues?
Risk based approach, time based?
Inspection techniques applied?
Failures reported? Contributing elements?
Detailed sharing would support.

The basis are the same as defined in API 583.
Appendix A.4 provides the details to arrive at the likelihood score and accordingly an inspection plan is formed.
Initially, we had 5 yearly CUF survey at our site which was then converted to a risk based approach.
The zones have been defined now based upon the likelihood however in cases where there are changes in the external environment like an active steam /condensate, water leaks, splash screens have been applied with listed 3 monthly inspection till the leaks are addressed.

Apart from having an inspection program in place to identify any potential corrosion under fireproofing, regular screening in the area for any changes is important.
We had a situation with active water, condensate, and steam leaks nearby foreproofing locations or at the same location where the structures were fireproofed and result was unexpected damages leading to painting damages & structural corrosion.
A4 Appendix of API 583 is lot clear when a standalone inspection plan is required to be established.

We have implemented a plan for fireproofing inspection supported with a site map indicating locations of fireproofing and likelihood. The assessment for likelihood has been made according to A4 API 583 and based upon the same whenever an inspection is made, data is entered and next maintenance order is triggered (with change in next inspection date, if applicable). By default all the maintenance orders are set to 5 years frequency to inspect fireproofing.
