What is the criteria to identify, report, and investigate any activation of safety instrumented system?
Should all the cases be reported, and investigated?

Coming towards the identification, that comes directly through Operations and the investigation mainly depends on the criteria established at the site level.
Majorly related to the potential and actual severity, process safety incident investigation management system should be in place to handle that part.
Yes, all the cases which are related to either prevent or mitigate LOPC are required to be reported, and based upon the incident management system be investigated.

Exactly, the investigation part and the criteria should be outside API 754 and if needed should be called from Company's Incident Management System.
Some of my colleagues have confirmed that all SIS with prevention or mitigation of LOPC are required to be investigated within a month time, as per their site procedures.
I understand none of Tier-3 incidents should be left out from investigation.

It is the Company's incident management system to include the criteria when an activation of SIS is required to be investigated and what are the other terms of references.
On an actual activation of SIS related to LOPC, investigation should be made. I confirm that is recordable event supported with an investigation report at our facility.
SIS activations not leading to prevent or mitigate LOPC but having heavy asset damage potential or major operational upset are also considered for investigation.
And not only this, I would say that Tier-3 events (all of them) after a careful review of its potential and actual severity as per Company's incident management system must be investigated.
