Part of an incident investigation team to support from process /operations side on loss of primary containment incident. Can someone share some pointers where and show should I be adding value?

This should be kept as a pocket guide while performing any investigation.
I like the way the identification of protection systems has been referenced.
Why tree has been given some very good recommendations as well.
