Design of Test Blinds

We have to carry out successive hydrostatic tests at our site for which test blinds have to be readily designed and arranged.
Please share some straight forward procedure for designing of test blinds. Any excel file would be very helpful.

Welded test blinds or bolted?
And the design code for your piping?

Both welded and bolted. Where we would be finding flanges obviously we will go for bolted blind flanges.
Code is for process piping ASME B31.3

For welded test blinds, use the simple formula as given in UG-34:

t = d * sqrt[(C*P)/(S*E)]

where,
t = thickness required
d = internal diameter of piping
C = attachment factor, take 0.33 and always weld with groove
S = allowable stress of the plate material
E = Joint efficiency, look what you decide about radiography
