And where else should we be looking for similar damage to occur?
Should inspection go beyond piping to pressure containments including vessels, exchangers?

Our inspection team says that there are local damages, fretting phenomena happening.
And how that is happening, and why recently with such recurring leaks, we are all looking into.

You would not find this weitten anywhere that you need to have such a check or an audit process in place but some sites have practice of regular inspection of their PSI package. I have seen some performing detailed line walk downs every 5 years.
