Re: Shift Handover Procedure - Mandatory items
The real issue which we have identified in having an effective handover is the agenda and conformance to that.
Recently, we expanded the agenda based upon the audits made reflecting to discuss a few more items.
Once they started discussing them, compliance was good in the start but after a few days ...

Re: Pressure Relief Device Activation
Correct that is jeem.
Yes, API 585 focuses on the part of the failure actually occurred on the pressure equipment and its investigation.
Surge in pressure means an actual demand, what triggered that should be investigated in all cases. We do not have a controlled practice at our place to do so but ...

Shift Handover Procedure - Mandatory items
When it comes to shift handover, procedure says a lot of things and then we face the reality which is not in conformance with whatever we agreed while writing the procedure. This has been concerning and it requires to be addressed in a smarter way making sure that the critical items are never missed ...

Re: Pressure Relief Device Activation
API 585 is the right reference when deciding about an event related to a pressure equipment integrity concern.
Once determined that there has been a failure of pressure relief device resulting in a loss of product or an actual overpressure scenario, guidelines provided in API 585 can be applied. As ...

Re: Identification of CCVs
See Class 1 check valves as classified by Shell are the ones upon failure to restrict the reverse flow can result in an overpressure scenario upstream.
This is what we learnt as part of their DEP 80.45.10.11.
But check valves which upon failure can result in process chemistry detrimental for the ...

Re: Dynamic Risk Assessment
I will take this as well.
Dynamic risk assessment is connected with identification of a hazard/s once the job is in-progress.
We opened up a heat exchanger that was planned to be cleaned with hydrojetting however, the deposits were not getting cleaned and the pressure was being increased. It was ...

Yes the similar practices as ivani1 has explained for routine activities subjected to a review by concerned. A complete list of maintenance routine activities are supported with HIRA which are validated by Maintenance Engineers, reviewed by HSE Engineers, and then made available for use.
