I think this has happened due to rapid cooling after the weld metal has been deposited & sufficient hydrogen has been absorbed by the weld metal from the arc atmosphere.

we've gone through the same situation in our plant, after the pre-comessioning (water flushing/air blowing & steam blowing) phase successfuly completed, we have done an internal inspection only for the critical lines by using Baroscopic device
