Now if you consider the documentation part, like operating and maintenance procedures, that would be tagged as leading indicators in Tier-4.
But PSI package itself, I would agree with neo, should be a lagging indicator if found with gaps.
What sort of gaps are you expecting to have within that?

What are the major causes of frequent pitting corrosion failure of carbamate pumps?
It is in 25-22-2 metallurgy and lately been reported with these failures on frequent basis.

You need to know:
There is NO universal Reynolds number limit for vibration
Turbulence is usually DESIRED - note this
The real limit is critical velocity vs actual velocity

Therefore, you may need to look into:
Reducing baffle spacing
Increasing tube support
Using rod baffles / helical baffles ...

This should be calling for an assessment comparing to what has been mentioned in the drawings & documents.
Which kind of document should be referred to in this case, though? Fireproofing Protection Philosophy like irish mentioned. HSE could help here to get these requirements verified.

There is a cost advantage as well. U-tube exchangers are often cheaper because of only one tubesheet (fixed has two). There are no expansion joints needed (which are expensive + failure-prone). Fabrication is simple, so if service allows, it can save you on budget.
